Pre-check before using the adult name change form

Ontario’s "Application to Change an Adult’s Name" form is for individuals aged 16 and over who have lived in Ontario for at least 12 months prior to submitting the application; those aged 16–17 require additional parental consent. This is a legal name change, which is different from simply using a spouse’s surname after marriage.

The guarantor for this application is different from a Canadian passport guarantor. Clients must read the form's criteria carefully and make their own arrangements within the scope of the listed package.

KeyApply assists with completing legal name change applications in Ontario for clients who need guidance in Vietnamese. This service package covers one Application to Change an Adult’s Name, based on the information and documents provided by the client.

Service fees and estimated costs

KeyApply application preparation fee: 150 CAD/application, excluding applicable taxes.

Government fees are to be paid separately by the client. Application submission/shipping fees are also to be paid separately by the client.

• Estimated Ontario adult name change fee: 137 CAD, based on government rates at the time of submission; some cases may be fee-exempt.
• Total estimated service and government fees: 287 CAD, excluding taxes, shipping, and additional costs.
• Estimated shipping budget provided by KeyApply: 15–35 CAD/shipment within Canada; actual fees depend on the address, number of shipments, and delivery speed. Receiving the completed form via email incurs no shipping fees from KeyApply.
• Guarantor, swearing/affirming signature before an authorized official, translation, background checks, and reissuing of other documents if required: the client must make their own arrangements and pay fees directly to the service provider.

Application package includes

• Guidance on providing the necessary information to complete the form.
• Completing the applicant's sections based on the answers and documents provided by the client.
• Reviewing the completed sections to minimize missing information or input errors.
• Sending a draft for the client to review, confirm, and adjust information before signing.
• Sending the completed application with instructions on sections for the client to sign and sections to be completed by an authorized official as per the form requirements.

Client preparation requirements

• Identification documents, birth certificate, and status information in Canada as required by the application form.
• Current name, desired name, address, and residency history required for the declaration.
• Information regarding previous name changes and marriage-related documents if required by the form.
• Full, accurate answers to the application questions; additional documents depending on the individual case.

Scope and responsibilities

KeyApply does not provide guarantor services and will not sign the "Guarantor’s Statement" in this package. Clients must arrange for their own guarantor and authorized official to witness the swearing/affirmation of their signature if required. KeyApply only completes the information; we do not sign on behalf of the client or the authorized official.

The package does not include application submission, follow-up with the agency, legal advice, notarization, translation, or updating of passports, PR cards, and driver's licenses after the name change. Applications for minors, multiple applications, or other procedures require a separate quote before commencement.

Clients must review and confirm all information before signing and submitting the application themselves. Processing times and name change decisions are at the discretion of the authorized agency. KeyApply is a private support service; clients may choose to complete and submit the application themselves.
