Below is a preliminary questionnaire for consulting with the Principal Applicant for Canadian Investment and Entrepreneur programs.

2. What is your highest level of education? Where did you study?
3. Do you hold an English or French certificate? (IELTS/CELPIP/TEF/TCF, etc.). If yes, what is your current score and test date? If you do not have a certificate, how would you rate your listening-speaking-reading-writing ability? (basic/intermediate/advanced/fluent)
4. What is your estimated total net worth in CAD? (Assets minus liabilities)
5. What form are these net assets primarily in? (cash/real estate/company shares/securities/other)
6. What is the primary origin of your assets? (business/salary/investment/inheritance/other)
7. Can you prove your assets and source of funds with financial records/tax documents/contracts? (yes/partially/difficult)
8. Approximately how much CAD do you intend to invest in Canada?
9. How do you intend to invest: by starting a new business or purchasing an existing one?
10. What industry do you wish to operate in Canada? (prioritize 1–2 industries)
11. Do you have experience as a business owner or in senior management? For how many years?
12. What was your highest role in the last 5–10 years? (owner/co-founder/CEO/GM/department head, etc.)
13. What was the scale of the business you operated: approximately how many employees?
14. What was the estimated average annual revenue of the business you operated?
15. Can you provide documentation to verify your management/business ownership experience? (business registration/licenses/tax reports/bank statements/contracts/payroll, etc.)
16. Are you willing to move to Canada full-time for 2–3 years to directly operate the business?
17. If yes, when do you plan to move and to which province (Ontario/Nova Scotia/etc.)?
18. Who will be accompanying you? (spouse, children's ages)
